• Address, telephone, email
• Dates of Production
• Total Number of Performances
• Title of Play
• Author of Play
• Book Title in which play can be found (if not a standalone play)

For professional performance applications we will need:

• Name of Applicant
• Name of Producing Organization
• Address, telephone, email
• Seating Capacity
• Ticket Price
• Dates of Production
• Total Number of Performances
• Title of Play
• Author of Play
• Book Title in which play can be found (if not a standalone play)
